Revolutionizing Drug Discovery: Progress and Future Horizons in Immune-Competent Human Skin Disease Models

Abstract

Skin diseases, characterized by a significant immune component, present a formidable obstacle in drug development due to disparities in animal and human immunology. This extensive review explores the realm of immune-competent human skin disease models, emphasizing their pivotal role in advancing drug discovery. Addressing prevalent issues in current models, such as inadequate prediction of human immune responses, the review navigates through innovative in vitro alternatives, with a focus on co-cultures and 3D organotypic systems. Noteworthy diseases including fibrosis, autoimmune disorders, psoriasis, cancer, and contact allergy are scrutinized, providing a detailed analysis of pathology and available models. The review underscores the limitations of existing models, emphasizing the imperative for superior non-animal, human immune-competent, and scalable skin disease models featuring comprehensive biomarkers. Furthermore, attention is directed towards areas requiring future development, including the integration of diverse immune cells and scalability. The urgent need for accurate drug discovery methodologies aligning with ethical guidelines, such as the 3Rs (reduction, refinement, and replacement of animals in experiments), is highlighted.
